Act now! The deadline for nominations is March 13.
As a San Bernardino County Medical Society member, you are invited to participate by nominating a fellow member or citizen you feel qualifies for an award. Every award is not necessarily bestowed each year, and regardless of the qualifications of the individual, under no circumstances will the same award be given to the same individual in any subsequent year.
Announcement and nomination forms were faxed to members on Feb. 7. If you did not receive the fax, please contact Linda Sue Myers at 951/787-7700, ext. 105, and request a copy. Entries must be submitted on an official "Nomination Form." Please duplicate the form for multiple nominations. You may also visit www.sbcms.org to download a form. Go to www.sbcms.org/news/outstandingawards/history.htm to view previous award recipients.
Send your nominations to SBCMS, P.O. Box 11029, San Bernardino, CA 92423-1029, or fax 951/787-8795. If you have questions, call SBCMS at 951/787-7700 or 909/825-6526. The deadline for nominations is March 13, 2006.
Winners will be selected at the SBCMS Executive Committee meeting on March 27, and the awards will be presented in June at the Installation of Officers meeting.
